Regardless of the disability, we aim to build capacity of an individual so they can live a life based on their strengths, rights and choices.
This program includes services of mentoring, peer-support, and individual skills development. Choices4u mentoring program support team understands the importance of this service in their plan and works with participants and other stakeholders, including Allied Health Professionals and therapists, to co-design and develop relevant activities to improve participants’ capacity. Choices4u mentors are skilled at providing NDIS coaching program skills to build participants’ independent living skills, including domestic tasks, tenancy maintenance, social skills, and emotional regulation skills. The mentoring program is based on participants’ inspiration, interests, and goals for working with (and not for) participants to acknowledge the ownership of action, build positive relationships, and break down goals into manageable steps or tasks to practice regularly. Choices4u monitors the progress of the mentoring program closely and explores better ways or approaches to support participants in achieving their goals without giving up.
Participants can utilize the funding of increased social and community participation (CB social community civic) to access this program.
