Regardless of the disability, we aim to build capacity of an individual so they can live a life based on their strengths, rights and choices.
Community access is also interpreted as social and community participation in a participant’s NDIS plan. Choices4u explores participants’ individual and unique interests and links them to the right support workers who have relevant experience or interest to support participants attending social and community events. Community access support includes a range of services to assist participants with grocery shopping, attending medical appointments, and engaging in meaningful and entertaining activities such as visiting an art gallery, zoo, fishing, or walking in the park or along the beach.
In this service, transport support can be challenging for some participants or tricky due to a lack of transport funding in the plan. Choices4u is willing to work with participants in exploring a strategy or solution to overcome this barrier and maximize participants’ opportunities to connect and engage with the community.
Participants can utilize the core support funding in their plan to access this service.
